If you are enrolled in a Primary Care Clinic at the Milwaukee VA or a Community Based Outpatient Clinic (CBOC): Call your Primary Care Clinic Social Worker and ask for a consultation for Homemaker Home Health Aide (HHA) (414) 384-2000.
If you do not get your medical care from the VA, you must first enroll in VA Health Care and ask for an appointment in a Primary Care Clinic. At your first Primary Care Clinic appointment, you can ask for a consult for Homemaker Home Health Aide.
Not all Veterans are eligible for enrollment in VA Health Care. If a Veteran is eligible, assignment to a Primary Care Clinic may take several months. VA payment for HHA services cannot be authorized until the Veteran meets all the eligibility criteria and has received written approval from the HHA Program Coordinator.
